The provision of Direct Primary Care Medical Services provided by Simmons Family Medicine, is not subject to regulation by the Texas Department of Insurance; is NOT a contract for health or accident insurance; and does not provide accident coverage. It will only cover the services as described in the DPC Membership Agreement. Patient should obtain or keep such health insurance policy(s) or plans that will cover Patient for general healthcare costs.
